I do not think you should just assume people want to play the "Arcade Cores" with a gamepad. Yes i assume. There is also no such thing as "Arcade controller". Any digital joystick with 2-6 buttons can be called as arcade controller. MiSTer is fully compatible with digital joysticks with enough butt...

I would dig into internals to see if possible to get a raw signals from inside. It's definitely possible for buttons, analog sticks. This is too overcomplicated for a controller adapter. A single Teensy can do the whole job. It's only a matter of available GPIOs. Teensy 3.2 has great amount of GPIOs. May be it's related to P1/P2 control? Could be. I checked a little bit the history of this thread it seems to me that loads of people have issues with controllers on the core which are similar or related. (load ms pacman first to get select and start recognized... etc...) What seems to work for me ...

I found a spare encoder in a drawer. It's a 2 players, I tried it and it works... But it works for 1P only ! That's because the MiSTer joystick mapping system is made for 1P only I guess. I mean, it only asks to press for 1P buttons, otherwise I guess it could work. it works with 2 players on cores...
