At this point, since you've already stripped the TT to barebones, inspect both sides of the board with a magnifying glass if possible. Pay special attention to the solder side and look for bad/cold solder joints, possible previous rework crud (clean flux residue with IPA), check tracks for corrosion...

TOS 3.05 patches were added to TOS 3.06. I'm pretty sure there are no official patches to 3.06. There are PAK patches though. Also, read this for more patches: https://forum.atari-home.de/index.php?topic=13347.0

Gratz. Nothing essential, really. Unless you only plan to run on TOS and not MiNT or MagiC it's not worth patching the TOS. A patch worth doing though is what the boys behind Thunder and Storm have done. They've adapted TOS 2.06 IDE boot ability to their TOS 3.06 version that ships with Thunder and ...

The TOS 2.06 decoding works and the MACH210 is 12ns. I want to rule out bad SIMMs before I start looking at the wiring.

You almost certainly need the -5V in that case. It was required on some ISA gfx and sound cards back in the day.

This is exactly the same converter from the same seller I have been using on my TT and PicoPSU for almost a year. Highly recommended.
